Prior to coming in for your fist appointment please review (if desired print off for your records) the HIPAA Notice and Office Policies. You will then need to print off, complete and bring with you the Registration Information form, HIPAA Signature form and Event of Breach Authorization form. The Release of Information form only needs to be completed if you need records sent or received from someone else. Thank you.
